Each scale page explains what the scale measures, how to score it, how to read the result, common mistakes and a worked example, and it can be printed as a one-page sheet.
How we build each page
- Items, points and cut-offs come from citable sources: the original publications, US government agencies (NIH/NINDS, AHRQ, CDC, NIDA), professional bodies and peer-reviewed or open nursing textbooks. The sources are listed at the bottom of every page.
- When sources disagree, we show the most cited US version and describe the alternatives, so you can match the version your facility uses.
- Copyrighted wording is paraphrased. Some scales (for example, the Braden Scale and FLACC) are protected; we describe each option faithfully in our own words and point you to the official form.
- Explanations and example cases are written by our editorial team in plain language, and every calculator is tested against its minimum and maximum score.
- Pages are reviewed and dated; the current review date is September 30, 2026. The full process is in our editorial policy.
What this site is not
Nursing Scales is an educational resource. It is not affiliated with the authors or owners of any scale, with any hospital, or with any licensing body. A score supports your assessment; it never replaces clinical judgment, a licensed provider or your facility's policies and protocols. Do not use this site in an emergency.
